There are a number of services locally that you are able to make self referrals to.

GP referrals
To request a GP referral:

Provide as much information as you can as this will help us to process your request.
Specialist and hospital care
If a GP or another member of our healthcare team believes that you need hospital treatment or specialist care elsewhere, they will discuss this with you and arrange the referral via the NHS e-Referral system. Once the referral has been requested we will then write to you enclosing instructions on how you can book the appointment with the hospital or specialist.
Tracking a referral
To track an existing referral:

NHS e-Referral
If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system. Once the referral has been requested we will then write to you enclosing instructions on how you can book the appointment with the hospital or specialist.
You will receive a letter from us within 2 weeks.
